Sending a referral
Referrals are accepted by fax. The practice is accepting new patients and a referral is required for every consultation.

- Shortness of breath
- COPD
- Asthma

The form is fillable. Open it, complete it on screen, print it, and fax it to 705-478-8711.
A typed consultation request sent by fax is also accepted.
Investigations
Arranged, not performed onsite
Tests are not performed at the Algonquin Avenue office. The office arranges the following and confirms the location with the patient.
- Pulmonary function testing
- Methacholine challenge testing
- Elective bronchoscopy
- Lung imaging
- Polysomnograms
